PCN Clinical Team

All Care homes have been allocated a dedicated clinician whose role will include overseeing the weekly ward rounds, initiating, and reviewing personalised care plans, completing chronic reviews, implementing advanced care planning and much more. Physician Associates and Advanced Clinical Practitioners attend the monthly MDT and if necessary, present any resident cases.

 

The Role of the Physician Associate

As of January 2025, our Care Home team includes 2 Physician Associates and 2 Advanced Clinical Practitioners.

Physician associates are medically trained, generalist healthcare professionals, who work alongside doctors and provide medical care as an integral part of the multidisciplinary team. Physician associates are dependent practitioners, who are supported by clinical leadership.

Each clinician has protected supervision time with a GP at the end of their session. This ensures that they can discuss residents and if necessary, get input from the doctor before making any clinical decisions.

Physician Associates support doctors in diagnosing and managing patients. They are trained to perform several roles, including taking medical histories, performing examinations, analysing test results, and diagnosing illnesses.

 

The Role of Advanced Clinical Practitioner

Advanced clinical practice is provided by skilled and registered health and care professionals. This practice involves a high level of independence and complex decision-making. It requires a master’s degree or an equivalent qualification that covers four main areas: clinical practice, leadership and management, education, and research. Practitioners must show key skills and specific clinical knowledge in their area of expertise.

Advanced clinical practice means working together with individuals, families, and caregivers to manage clinical care. It involves understanding and solving complex problems in different settings. This approach helps create new solutions that improve people’s experiences and health outcomes.

PCN Pharmacy Team

As a Care Home Service, we work in direct collaboration with our in house pharmacy team on a daily basis. We ensure that structured medication reviews are referred to our highly skilled pharmacists in line with the EHCH and IIF framework and in turn they advise when birth month review have been completed in line with their protocols.

Care homes often request medication reviews on the routine ward round list, these residents are then referred directly to the pharmacy team, bypassing the need for the PA to review the patient unnecessarily.
