We believe that people living in care homes should receive the same high-quality healthcare support as they would if they were living in their own home.
Healthier West Wirral Primary Care Network (PCN) works closely with care homes to provide enhanced primary care and access to specialist services. Our aim is to help residents stay as healthy and independent as possible, receive the right support at the right time, and reduce avoidable hospital admissions where we can.
We support around 800 residents across 24 CQC-registered care homes in West Wirral. We work in partnership with care home teams, community services and local organisations to deliver joined-up, personalised care.
Our Multi-Disciplinary Team (MDT) meetings bring together professionals from a range of backgrounds to support person-centred planning and coordinated care.
