Care Home Services
It is our belief that people living in care homes should expect the same level of support from the PCN as if they were living in their own home.
Healthier West Wirral is working hard to ensure people living in care homes have access to enhanced primary care and to specialist services and maintain their independence as far as possible by reducing, delaying, or preventing the need for additional health and social care services.
Our service provides care to 800 residents across 26 care homes. We maintain excellent relationships with community services and local organisations, ensuring a collaborative approach to personalised care for each of our residents.
Our monthly MDT includes a mixed range of professional backgrounds, adding to the knowledge pool available for each person-centred discussion.
